A ROLE OF ULTRASOUND GUIDED FINE-NEEDLE ASPIRATION BIOPSY IN THE DIAGNOSIS OF ABDOMINAL TUMORS

Background: Over the last ten-year period ultrasonography (US) has been increasingly applied for both diagnostic and therapeutic purposes. Ultrasound examination marks a great step forward in noninvasive diagnostics but it is often unable to differentiate between benign and malignant focal changes. The paper is aimed at presenting our experience and results with US guided fine-needle aspiration biopsy (FNAB) of upper abdominal lesions, particularly malignancies. Methods: We have used the ultrasound instrument Siemens Sonoline SL 2 with a commercial equipment, a sector probe of 3.5 MHz with a guide and fine needles of 0.6-0.8 cm in diameter. A targeted ultrasound guided fine-needle aspiration biopsy (FNAB) was performed in 210 patients with a lesion of an upper-abdominal organ. The patients included in the trial had a liver (151) or pancreas (42) disorders, enlarged lymph nodes and other unclear tumor signs (17) in the upper-abdomen. Malignant liver disorders were the major pathology, including primary tumors of the liver and metastatic liver lesions. Results: Targeted US-guided FNAB were performed in 210 patients. Due to suspect liver lesions a targeted US-guided FNAB was performed in 151 patients, confirming a malignancy, either primary or metastatic, in 108 patients. Out of them, a malignant primary neoplasm was found to be involved in 80%, while 20 % had a metastatic liver involvement. The accuracy of the method in differentiating between malignant and benign lesions has been found to be 81.4%, accompanied with the sensitivity of 93%. Conclusion: The obtained results reveal a high percentage of positive cytologic findings, regardless a malignant or a benign lesion was involved. |
