CYTOLOGIC DIAGNOSIS OF PRIMARY LUNG CANCER IN SPUTUM AND BRONCHIAL WASHING SPECIMENS: ACCURACY IN CYTOTYPING AND PROBLEMS

Background: Cytologic examination of sputum and bronchial washings are important methods in the diagnosis of primary lung cancer patients. In recent years clinicians have a tendency to base therapeutic management on the results of cytology. The present study was undertaken in order to analyze the results of cytodiagnosis of primary lung cancer in sputum and bronchial washing specimens, as well as to investigate the accuracy of cytologic typing with respect to the histologic diagnosis. Methods: Patients from Theagenion Cancer Hospital were included in the study and the analysis was based on the type of carcinoma and sex of the patient. Results: In 81.8 % of the cases there was an agreement between cytology and histology in the diagnosis of the type of carcinoma. Diagnostic accuracy was higher in well differentiated squamous cell carcinoma and small cell carcinomas. In some cases with histologic diagnosis of poorly differentiated carcinomas, cytology exhibited higher tumor typing accuracy identifying a degree of cellular differentiation. The factors which influence the classification of carcinomas are also discussed. Conclusion: Cytologic examination has a high diagnostic value in cases of primary lung cancer, and therapeutic management can be based on cytodiagnosis. |
