THE IMPORTANCE OF PERIRECTAL LYMPH NODES INVOLVEMENT IN THE MANAGEMENT OF ANAL CARCINOMA

To evaluate the incidence, the treatment and the prognostic impact of perirectal lymph node in anal canal carcinoma (ACC). Between 1983-1993, 95 patients with ACC were treated in pilot protocol combining Radiotherapy and 5FU CDDP A perirectal lymph node metastasis was found in 36 patients (38%) either by digital rectal examination (DRE) or endorectal sonography (ERS). The 5 year overall survival was 84% for the entire series. It was 88% vs 76% for No or N1-3 patients ERS was more sensitive and more specific than DRE to detect perirectal lymph node metastasis.
An accurate assessment of the lymph node of the mesorectum is essential for a good staging of ACC. Concomitant Radio-Chemotherapy is able to control these lymph nodes in most of the cases. |
