- METACHRÎNOUS HEPATOCELLULAR CARCINOMA IN A PATIENT WITH TREATED
COLORECTAL CARCINOMA - CASE STUDY

ABSTRACT
To
achieve longer and quality life of a patient treated for colorectal
carcinoma it is important to carry out regular control after active
treatment. According to the recommendations of the National Guide
of Clinical Practice (EPOS Health Consultants) clinical, radiographic
and laboratory examinations should be done less frequently and only
in case of patients with suspicious symptoms, with ultrasonographic
control every six months. The case of the patient R.K. shows that
it is not advisable to stick to the rules by all means. Thanks to
regular three-month controls that were made during the period of
three years after completed active treatment of colorectal carcinoma
(operation, adjuvant chemotherapy) a deviation in the GGT findings
(increased level of gamma glutamyltransferase 10% above normal level)
and solitary change in the right liver lobe were detected, while
other findings were within reference limits. Surgical treatment
of liver metastasis was indicated and the final histological findings
showed hepatocellular carcinoma. Metachronous liver tumor of different
histology is a rarity. In order to minimize the factor of surprise,
it would be necessary prior to operation to introduce the control
by tumor markers CEA and alpha-fetoprotein, and to consider the
possibility of primary liver tumor; thus a more radical type of
operation could be chosen. Such a treatment would surely have influence
to the length and quality of the patient's life. |
