COMBINED TREATMENT OF LOCALLY ADVANCED RECTAL CARCINOMA

  ABSTRACT
To check the value of preoperative radiotherapy in the decrease of frequency of local recurrence of locally advanced rectal carcinoma. Between June 2000 and July 2003 in all patients with histologically proven adenocarcinoma of rectum (up to 12 cm from anal verge) contrast magnetic resonance scan of pelvis was done. In the cases of T3/T4 tumors preoperative radiotherapy (two fields technique AP/PA, 50 Gy, daily fraction 2 Gy) was applied. Six weeks following the end of radiotherapy new magnetic resonance pelvic scan and surgical procedure with total mesorectal excision was done. Forty-one patients were treated (28 males and 13 females) with median age 63 years (range, 42-77 years). Average distance of the tumor from anal verge was 4.8 cm. Six patients (14.6 %) were still inoperable after radiotherapy (two developed distant metastases and in four cases tumor locally progressed). In 35 patients (85.4%) radical operation has been performed (18 low anterior resections and 17 amputations of rectum). Median time of follow up was 19 months and in four cases (11.4%) local recurrence developed 8 to 27 months after Miles operation in all cases. preoperative radiotherapy combined with radical surgery for locally advanced rectal tumors provides high grade of operability.
