ABSTRACT
Early
diagnosis and immediate surgical treatment in early stage of tumor
(melanoma in situ, "thin" melanoma) are the aim, which all surgeons
try to achieve. Unfortunately, surgeons are encountered with complex
problems in malignant melanoma treatment today, regardless of knowledge
in malignant melanoma problems, as medical personnel, as an entire
population.
In this paper authors presented retrospective analysis of total
245 patients with malignant melanoma, surgically treated in last
five years on Clinic for plastic and reconstructive surgery, and
Clinic for maxillofacial and oral surgery, Institute of surgery,
Clinical center Novi Sad. According to sex, 130 (53,05%) patients
were female, and 115 (46,93%) were male. According to age, the most
number of patients were aged among 51-60 (30, 20%). Dominant localization
of tumor was back (51 patients, 20, 81%). Primary tumor was in 203
patients (82, 85%), primary tumors with metastasis in regional lymph
nodes were in 11 patients (4, 48%), and metastases in previously
surgical treated primary tumors were in 42 patients (17, 14%). Superficial
spreading malignant melanoma was the most numerous, according to
histological examination (99 patients, 48, 76%), and nodular melanoma
(89 patients, 43, 84%), as the second numerous tumor. According
to micro staging of tumor, Clark IV stadium of invasion were in
65 patients (32, 01%), and Breslow V thickness of tumor were in
104 patients (51, 23%). Metastases in axillar lymph nodes were in
25 patients (10, 20%), and cutaneous and subcutaneous metastases
were in 20 patients (8, 16%). Recurrence of tumor was in 2 patients
(0,81%), and same number of patients was with metastasis without
identification of primary tumor.
Increased incidence of malignant melanoma is evident in our paper,
and it is reflection of better detection and knowledge about tumor,
and a real increase of appearance of malignant melanoma in human
population. Alarming number of "thick" melanoma is presented in
our analysis, as a numerous tumors localized on back, which is prognostic
bad localization. Large number of patients came to surgery with
metastasis, thus decreased survival. Numerous of our patients previously
had inadequate treatment in other hospitals. This fact pointed out
insufficient education of physicians and medical staff about real
problems in differential diagnosis of malignant melanoma. |
