MALIGNUM DIFFERENTIAL DIAGNOSIS

ABSTRACT
Malignant
melanoma (MM) is one of the most aggressive cutaneous malignancy
and is high on the list of diagnoses which accurate identification
remains the single most important problem in pathology. Despite
rapid advances in modern diagnostic procedures, the diagnosis of
MM still hinges on morphological parameters. The standard and important
criteria for diagnosis of MM are summarized as ABCD features. The
pagetoid melanocytosis (PM) is very important especially in differential
diagnosis. PM is the typical upward migration of melanocytes which
has to fulfill those three histological criteria - the cells in
the superficial epidermis must be clearly identifiable as melanocytes;
they must not occur as a direct extension from the junctional component
but must appear discontinuous; and the melanocytes must be located
in the suprabasilar epidermis above the line parallel to the skin
surface, placed at the basal epidermal layer overlaying the most
superficial dermal papilla. In certain conditions, nonmelanocytic
cells in the superficial epidermis may be difficult to distinguish
from melanocytes. These include: pagetoid Bowen's disease, Paget's
disease, mycosis fungoides and epidermotropic Merkel cell carcinoma.
In great number of melanocytic lesions as well, the PM is evident,
as in Spitz nevus, dysplastic nevus, pigmented spindle cell nevus
of Reed, nevi of infancy and childhood and nevi of certain sites-
palms, soles and vulva. If the presence of PM is established additional
features in favor of malignant melanoma are: a diffuse and dense
extension over a wide area, prominent melanocytic atypia and free-floating
PM. |
